(2)具有良好的商業(yè)信譽(yù)和財(cái)務(wù)會(huì)計(jì)制度; (3)具有履行合同所必需的設(shè)備和專(zhuān)業(yè)技術(shù)能力,投標(biāo)單位***B類(lèi)維保工程能力(能力要求參見(jiàn)DB32/T 4696—2024); (4)投標(biāo)人須指派項(xiàng)目負(fù)責(zé)人1名,需具備相應(yīng)的注冊(cè)消防工程師資格。固定1—2名責(zé)任心強(qiáng)、業(yè)務(wù)過(guò)硬和無(wú)違法犯罪記錄的技術(shù)負(fù)責(zé)人,負(fù)責(zé)消防設(shè)施維保、檢測(cè),操作人員應(yīng)當(dāng)通過(guò)消防特有工種職業(yè)技能鑒定,持有中級(jí)及以上職業(yè)資格證書(shū)。 (5)近3年內(nèi)有相關(guān)成功業(yè)績(jī)1例以上(有效業(yè)績(jī)的定義:①合同為同類(lèi)型相關(guān)項(xiàng)目;②須提供該業(yè)績(jī)合同的復(fù)印件(包括但不限于合同、雙方蓋章頁(yè)、總金額***提供該業(yè)績(jī)合同的項(xiàng)目驗(yàn)收證明(如:業(yè)主證明或完工證明、驗(yàn)收?qǐng)?bào)告等)或該業(yè)績(jī)合同的所有收款憑證,以證明除驗(yàn)收、質(zhì)保外其他階段付款均已支付,(該業(yè)績(jī)合同的項(xiàng)目驗(yàn)收證明或該業(yè)績(jī)合同的所有收款憑證可二選一提供);④所有證明文件在必要時(shí)需提供紙質(zhì)原件備查。) (6)環(huán)境管理、職業(yè)健康及安全管理實(shí)施情況及績(jī)效說(shuō)明(擁有***01體系證書(shū)可免于提供); (7)近三年發(fā)生如生產(chǎn)安全事故,環(huán)保事故、分包商員工職業(yè)健康問(wèn)題等被有關(guān)部門(mén)進(jìn)行行政處罰或有異常信用記錄的,應(yīng)提供書(shū)面分析改進(jìn)報(bào)告。(未如實(shí)陳述者,我司有權(quán)取消中標(biāo)資格及其他處罰措施。) (8)當(dāng)招標(biāo)項(xiàng)目涉及國(guó)家行政許可要求時(shí),具有相應(yīng)的行政許可資質(zhì); (9)法律、法規(guī)規(guī)定的其他條件。 (10)存在行政處罰的單位,根據(jù)處罰情況招標(biāo)小組討論決定是否通過(guò)資格預(yù)審。 3.2資格預(yù)審提交材料清單: (1)投標(biāo)報(bào)名表(見(jiàn)附件1); (2)企業(yè)法人營(yíng)業(yè)執(zhí)照正副本復(fù)印件(具有統(tǒng)一社會(huì)信用代碼的營(yíng)業(yè)執(zhí)照,增值稅一般納稅人資格證明,代理經(jīng)銷(xiāo)商需同時(shí)提供自身及源頭生產(chǎn)廠家) (3)法定代表人、被授權(quán)人及聯(lián)系人身份證復(fù)印件及授權(quán)委托書(shū)(附件2); (4)企業(yè)情況簡(jiǎn)介; (5)近3年承擔(dān)的類(lèi)似相關(guān)()業(yè)績(jī)證明(合同復(fù)印件及對(duì)應(yīng)發(fā)票復(fù)印件或回款證明至少1份); (6)經(jīng)會(huì)計(jì)事務(wù)所審計(jì)的近三年財(cái)務(wù)會(huì)計(jì)報(bào)告或財(cái)務(wù)報(bào)表(***-***,包含利潤(rùn)表、負(fù)債表、現(xiàn)金流量表)。 (7)ISO***及其他管理體系證書(shū)(代理經(jīng)銷(xiāo)商需同時(shí)提供自身及源頭生產(chǎn)廠家證書(shū),若無(wú)管理體系證書(shū),需要填寫(xiě)序8); (8)環(huán)境管理、職業(yè)健康及安全管理實(shí)施情況及績(jī)效說(shuō)明(擁有***01體系證書(shū)無(wú)需填寫(xiě)); (9)近3年發(fā)生的生產(chǎn)安全事故的狀況描述,尤其說(shuō)明發(fā)生一般及以上生產(chǎn)安全事故的情況(文字說(shuō)明); (10)近3年的環(huán)保處置情況描述,尤其說(shuō)明發(fā)生的突發(fā)環(huán)境事件、受到環(huán)保主管部門(mén)處罰的情況(文字說(shuō)明); (3)貿(mào)易商(包括經(jīng)銷(xiāo)商和代理商)需具有固定的經(jīng)營(yíng)場(chǎng)所,經(jīng)營(yíng)產(chǎn)品質(zhì)量合格,具有本企業(yè)的ISO9001質(zhì)量體系認(rèn)證證書(shū)、ISO14001環(huán)境體系認(rèn)證證書(shū)、ISO45001職業(yè)健康安全體系認(rèn)證證書(shū)。并具有產(chǎn)品生產(chǎn)企業(yè)的代理協(xié)議或產(chǎn)品銷(xiāo)售授權(quán)資質(zhì);
