[endif]項(xiàng)目概況與采購(gòu)內(nèi)容 1.1采購(gòu)內(nèi)容：對(duì)湖北信通回收的OTT機(jī)頂盒、ONU設(shè)備、配件進(jìn)行分揀、統(tǒng)計(jì)、翻新；對(duì)檢測(cè)后無(wú)翻新價(jià)***機(jī)頂盒、ONU設(shè)備、配件處置前等相關(guān)工作，其他詳見(jiàn)技術(shù)規(guī)范書(shū)。 1.2服務(wù)周期：自合同簽訂之日起一年。 1.3標(biāo)包劃分：本項(xiàng)目不劃分為標(biāo)包。 1.4交付時(shí)間：自收到甲方訂單后20個(gè)日歷日完成。 1.5服務(wù)地點(diǎn)：***市。 1.6本項(xiàng)目設(shè)置最高參選限價(jià)******價(jià)***參選人報(bào)價(jià)***價(jià)***將被否決。 [if !supportLists]2. [endif]參選人資格要求 2.1本項(xiàng)目參選人應(yīng)為中華人民共和國(guó)境內(nèi)法律上和財(cái)務(wù)上獨(dú)立的法人或依法登記注冊(cè)的組織，合法運(yùn)作并獨(dú)立于比選人，且必須提供合法有效的營(yíng)業(yè)執(zhí)照。法人下屬不具備法人資格的分支機(jī)構(gòu)***，應(yīng)具備法人針對(duì)本項(xiàng)目或覆蓋本項(xiàng)目的經(jīng)營(yíng)事項(xiàng)的有效授權(quán)。 2.2參選人應(yīng)具備自******月***日至參選文件截止時(shí)間***。以單項(xiàng)合同或訂單（如提供訂單的，須同時(shí)提供框架協(xié)議）為準(zhǔn)，必須提供合同關(guān)鍵頁(yè)等。如提供框架或集中采購(gòu)合同，還須提供與之對(duì)應(yīng)的結(jié)算材料（包括采購(gòu)訂單、發(fā)票、結(jié)算文件或其他證明材料），并以訂單累計(jì)金額***或集中采購(gòu)合同金額***額內(nèi)。資料不全、評(píng)選委員會(huì)無(wú)法確定業(yè)績(jī)的不予認(rèn)可。（單項(xiàng)合同時(shí)間以雙方簽訂落款時(shí)間為準(zhǔn)，金額***金額***合同的時(shí)間以訂單時(shí)間為準(zhǔn)，金額***金額***2.3要求參選人為本項(xiàng)目配置的工作人員數(shù)量不少于5人，并提供身份證復(fù)印件、社保證明或勞動(dòng)合同，并承諾人員、工具等按工作需要合理配置（提供承諾書(shū)）。 2.4參選人中選后為本項(xiàng)目配備的翻新人員應(yīng)具備一定的安全意識(shí)及知識(shí)，相關(guān)手續(xù)、證件、保險(xiǎn)、安全設(shè)施、設(shè)備配備齊全，管理體系、制度完善，保證安全生產(chǎn)，***和投標(biāo)人自行承擔(dān)相關(guān)責(zé)任（提供承諾書(shū)）。 2.5參選人能夠或承諾向比選人提供增值稅發(fā)票（提供承諾書(shū)）。 2.6本次比選不接受聯(lián)合體參選，不允許轉(zhuǎn)包和違法分包。 2.7參選人的法定代表人或負(fù)責(zé)人為同一人或者存在控股、管理關(guān)系的不同參選人，不得參加同一標(biāo)段參選或者未劃分標(biāo)段的同一比選項(xiàng)目參選。 2.8參選人不得存在下列情形之一：（1）為比選人不具有獨(dú)立法人資格的附屬機(jī)構(gòu)（單位）；（2）被依法暫?；蛉∠稑?biāo)/參選資格的；（3）被責(zé)令停產(chǎn)停業(yè)、暫扣或者吊銷許可證、暫扣或者吊銷執(zhí)照；（4）進(jìn)入清算程序，或被宣告破產(chǎn)，或其他喪失履約能力的情形；（5）在最近三年內(nèi)（參選截止時(shí)間***）被相關(guān)行業(yè)主管部門(mén)或司法機(jī)關(guān)認(rèn)定騙取中標(biāo)/中選、嚴(yán)重違約、重大工程質(zhì)量或者安全問(wèn)題的；（6）在最近五年內(nèi)（參選截止時(shí)間***）被判處單位***，且行賄行為與采購(gòu)活動(dòng)相關(guān)的（以“中國(guó)裁判文書(shū)網(wǎng)”的生效判決為準(zhǔn)）；（7）在最近五年內(nèi)（參選截止時(shí)間***）被判處合同詐騙罪的（以“中國(guó)裁判文書(shū)網(wǎng)”的生效判決為準(zhǔn)）；（8）被最高人民法院認(rèn)定為失信被執(zhí)行人的(以“信用中國(guó)”網(wǎng)站（www***）或各級(jí)信用信息共享平臺(tái)公布的失信被執(zhí)行人名單為準(zhǔn))，已執(zhí)行完畢或不再執(zhí)行的除外；（9）被工商行政管理機(jī)關(guān)在國(guó)家企業(yè)信用信息公示系統(tǒng)中列入嚴(yán)重違法失信企業(yè)名單的；（10）***嚴(yán)重負(fù)面行為、特別嚴(yán)重負(fù)面行為清單的（11）法律法規(guī)、比選文件限定的其他情形。 [if !supportLists]3.
