(c)本項(xiàng)目的特定資格要求：3.根據(jù)《***市政府采購(gòu)供應(yīng)商登記及誠(chéng)信管理辦法》已登記入庫(kù)的供應(yīng)商。 4.其他資格要求： 1)具有獨(dú)立法人資格，其經(jīng)營(yíng)/業(yè)務(wù)范圍符合本項(xiàng)目要求； 2)參加政府采購(gòu)活動(dòng)前三年內(nèi)，在經(jīng)營(yíng)活動(dòng)中沒(méi)有重大違法記錄； 3)同時(shí)具有建筑工程總承包三級(jí)及其以上資質(zhì)或?qū)I(yè)承包建筑裝修裝飾工程二級(jí)及其以上資質(zhì)，且具備有效的安全生產(chǎn)許可證； 4)項(xiàng)目擬派項(xiàng)目經(jīng)理：建筑工程專業(yè)二級(jí)或以上注冊(cè)建造師執(zhí)業(yè)資格。該項(xiàng)目負(fù)責(zé)人以在***市建筑建材業(yè)網(wǎng)站上查詢?yōu)闇?zhǔn)，在冊(cè)且無(wú)在建項(xiàng)目記錄（同一工程相鄰分段發(fā)包或分期施工的除外）(磋商響應(yīng)文件遞交時(shí)不得調(diào)整，不得有在建項(xiàng)目)。 5)“投標(biāo)人 ”、“法人代表 ”及“項(xiàng)目負(fù)責(zé)人 ”近三年來(lái)無(wú)行賄犯罪記錄； 6)按照《關(guān)于在政府采購(gòu)活動(dòng)中查詢及使用信用記錄有關(guān)問(wèn)題的通知》（財(cái)庫(kù)〔2016〕125號(hào)）投標(biāo)人須具有良好的信用記錄； 7)本項(xiàng)目（□是?否）專門面向中、小、微型企業(yè)，事業(yè)法人等各類供應(yīng)商采購(gòu)。(殘疾人福利性單位、某某企業(yè)視同小型、微型企業(yè)。)如供應(yīng)商為中小企業(yè)，且滿足《政府采購(gòu)促進(jìn)中小企業(yè)發(fā)展管理辦法》(財(cái)庫(kù)[2020]46號(hào))規(guī)定條件的，必須提供《中小企業(yè)聲明函》； (c)Specific qualification requirements for this program:3.
