margin-right:90.0000pt; size:595.3000pt 841.9000pt; layout-grid:15.6000pt; mso-header-margin:42.5500pt; mso-footer-margin:49.6000pt; } div.Section0{page:Section0;}StartFragment ******省分行運營中心***上半年食堂食材及用品配送項目招標公告 1. 招標條件 ******省分行運營中心***上半年食堂食材及用品配送項目（招標人編號***標代理編號***，******省分行批準實施，項目資金來自企業(yè)自籌，資金已落實。******省分行，***（以下簡稱“招標代理機構(gòu)”）對該項目進行公開招標。 2. 項目概況與招標范圍 2.1 招標內(nèi)容：本項目擬通過公開招標的方式選擇1家供應商，提供***省分行運營中心***上半年食堂食材及用品（含食材、調(diào)味品、糧油等）配送服務(wù)。具體內(nèi)容及要求詳見招標文件***：項目需求書。 2.2標包劃分情況：本項目不劃分標包。 2.3 項目預算：20***元。 2.4 ******省分行出資。 2.5 合同有效期：自******月***日起至******月***日，如合同簽訂之日晚于******月***日，則合同有效期為自合同簽訂之日起至******月***日。 3. 投標人資格要求 3.1投標人須為具有獨立法人資格的境內(nèi)企業(yè)或其他組織（如是分支機構(gòu)，則須提供具備獨立法人資格的上級機構(gòu)***），對于已按“三證合一”登記制度更換新版營業(yè)執(zhí)照的（營業(yè)執(zhí)照、組織機構(gòu)***、稅務(wù)登記證三證合一），須提供由工商部門頒發(fā)的加載統(tǒng)一社會信用代碼的營業(yè)執(zhí)照。 3.2具有良好的商業(yè)信譽。須同時提供下列截圖：（1）國家企業(yè)信用信息公示系統(tǒng)（網(wǎng)址：http***cn/index.html）中投標人“列入嚴重違法失信企業(yè)名單（黑名單）信息”和“列入經(jīng)營異常名錄信息”的查詢結(jié)果截圖。（2）“信用中國”（網(wǎng)址：http***Page=4）“失信被執(zhí)行人”、“政府采購嚴重違法失信行為記錄名單”、“重大稅收違法失信主體”的查詢結(jié)果截圖；注：如投標人因單位***，無法提供上述截圖的，則須提供相應情況說明。 3.3 具有健全的財務(wù)會計制度，需提供***年度財務(wù)審計報告或***年度財務(wù)報表（包含資產(chǎn)負債表、利潤表及現(xiàn)金流量表）。注：因受成立年限影響，無法提供上述證明材料的，提供最近三個月任意一個月的財務(wù)報表。 3.4投標人必須具備***至今的同類項目合作經(jīng)驗。（以合同簽訂時間為準，要求提供合同關(guān)鍵頁復印件）。 3.5 資質(zhì)要求：投標人必須具備有效的《食品生產(chǎn)許可證》或《食品經(jīng)營許可證》。需提供相關(guān)證書復印件。 3.6項目關(guān)聯(lián)性：因“***省分行運營中心***上半年食堂后勤服務(wù)項目（簡稱：后勤服務(wù)項目）”和“***省分行運營中心***上半年食堂食材及用品配送項目（簡稱：食堂食材及用品配送項目）”關(guān)聯(lián)性較強，兩個項目不兼投不兼中（即每個投標人只能投標、中標其中一個項目，招標人優(yōu)先對“后勤服務(wù)項目”進行評標，如投標人同時參加“后勤服務(wù)項目”和“食材及用品配送項目”的投標，則只接受“后勤服務(wù)項目”的投標，“食堂食材及用品配送項目”的投標將視為無效投標）。投標人須提供承諾函。 3.7 投標人必須開具增值稅普通發(fā)票。注：投標人需提供稅局相關(guān)證明文件或已開具的增值稅普通發(fā)票樣本復印件。 3.8本次招標不接受聯(lián)合體投標。 3.9利害關(guān)系： 3.9.1單位***、管理關(guān)系的不同單位，不得參加同一標段投標或者未劃分標段的同一招標項目投標。須提供控股及管理關(guān)系情況申報表（格式見第六章）。 3.9.2投標人與郵政無投資關(guān)系且存在以下情況的，不得參加本次招標/采購活動：郵政領(lǐng)導人員及其親屬和其他特定關(guān)系人、郵政員工持股（***），以個人身份（組織委派的除外）擔任法人、董事長、總經(jīng)理、監(jiān)事的企業(yè)，以及郵政所屬工會或員工集體出資成立的企業(yè)。詳見投標人不得存在情況承諾書。 3.10投標人或其投標產(chǎn)品/服務(wù)不得存在下述情況： 3.10.1被列入招標人（***、******省分行）供應商管理黑名單，且在禁入期內(nèi)被暫?；蛉∠稑速Y格的。 4. 招標文件*** 4.1注冊登記凡有意參加投標者，須在中國郵政儲蓄銀行-郵銀易采平臺（以下簡稱郵銀易采平臺，http***）申請注冊（具體方式可詳見郵銀易采-幫助中心-供應商操作手冊），并在注冊成功后登錄賬號。 4.2購買文件 4.2.1招標文件***：******月***日至******月***日，每日上午***時***分至***時***分，下午***時***分至***時***分（北京時間，下同） 4.2.2招標文件***（￥500）整，售后不退。 4.2.3 招標文件***（有以下兩種方式，可選擇其一進行操作）： 4.2.3.1系統(tǒng)支付文件費用：***電子招投標平臺http***scm.cn完成招標文件***，***統(tǒng)一社會信用代碼，登錄后請及時完善信息并更改密碼。網(wǎng)站主頁可下載《中捷招標系統(tǒng)投標人網(wǎng)上售標操作手冊》參考流程。 4.2.3.2電匯/轉(zhuǎn)賬支付文件費用：將開票信息+營業(yè)執(zhí)照復印件蓋章+授權(quán)委托書+匯款底單以郵件方式發(fā)送給購買文件聯(lián)系人。購買文件聯(lián)系人：***，聯(lián)系電話：***，聯(lián)系郵箱：zhongjbss2@zjscs.com。 4.3獲取文件潛在投標人在郵銀易采平臺選擇采購業(yè)務(wù)-我的項目-，選定本項目，立即參與，進行購標（上傳文件費用付款憑證），經(jīng)審核通過后，潛在投標人即可在郵銀易采平臺（http***）平臺獲取電子版招標文件，可參與后續(xù)投標。 5. 投標文件的遞交本項目采用郵銀易采平臺電子投標和線下紙質(zhì)投標并行，投標人應在郵銀易采平臺遞交電子投標文件及線下遞交紙質(zhì)投標文件，否則視為無效投標。投標文件（電子投標文件和紙質(zhì)投標文件）遞交的截止時間***（投標截止時間***，下同）為******月***日***時***分（北京時間）。 5.1紙質(zhì)投標文件的遞交：遞交地點：***市體***路一號峰源大***樓會議室；逾期送達的、未送達指定地點***、不按照招標文件***，招標人將予以拒收。 5.2郵銀易采平臺電子投標文件的遞交：電子投標文件須通過郵銀易采平臺遞交（注：成功遞交電子投標文件的平臺狀態(tài)顯示為“遞交成功”）。逾期提交的電子投標文件，系統(tǒng)將不予接收。郵銀易采平臺電子投標文件須使用CA證書進行加密后遞交。CA證書具體辦理流程參見中國郵政儲蓄銀行-郵銀易采上方“CA辦理”專區(qū)。本項目投標文件的制作（包括簽章、加密等）、網(wǎng)上遞交、開標（解密、確認報價***件***、評標澄清問題答復等環(huán)節(jié)均需要使用CA證書。請有意向投標的供應商在注冊時同步申請辦理，以免影響正常投標。由于未及時辦理CA證書影響投標的，后果由投標人自行承擔。 5.3投標保證金：￥24,0***元（人民幣***元整）。 6.開標 6.1電子開標本項目將于投標文件遞交截止的同一時間，在***市體***路一號峰源大***樓會議室，使用郵銀易采平臺開標，邀請投標人參會代表攜帶電腦、WIFI熱點、CA證書以及生成的秘鑰文件等準時參加。 6.1當電子投標文件開標解密正常時，開標、評標以電子投標文件為依據(jù)。 6.2電子投標文件解密異常處理 6.2.1電子投標文件解密異常時，招標人/招標代理機構(gòu)***，若為系統(tǒng)故障原因，則推遲該項目的投標截止時間***（具體時間另行通知）直至該投標人完成電子投標文件解密。因投標人原因（如解密密碼輸入錯誤等）造成投標文件未解密的，則視為撤銷其投標文件。 6.2.2若因系統(tǒng)故障無法及時解決，則啟用紙質(zhì)招標投標程序。 7. 資格審查方式本項目采用資格后審辦法對投標人進行資格審查，由評標委員會在開標后按照招標文件***。 8. 公告發(fā)布本公告或公告的修改、補充同時在中國郵政儲蓄銀行郵銀易采平臺（http***）、采購與招標網(wǎng)（http***ding.com.cn）、中國郵政官方網(wǎng)站（http***t.com.cn）、***電子招投標平臺（http***scm.cn）、中國金融集中采購網(wǎng)（http***m）以及中國招標投標公共服務(wù)平臺（http***rvice.com）上發(fā)布。本公告在各媒體發(fā)布的文本如有不同之處，以在中國郵政官方網(wǎng)站發(fā)布的文本為準。 9. 聯(lián)系方式招標人：******省分行地址：***市***區(qū)體***路1號峰源大廈招標代理機構(gòu)：*** 地址： ***市較***路21號郵編： 510055 項目聯(lián)系人***李威、黃慶生、王菲、湯茗淇、鄺炎瀅、謝知、施其鏗聯(lián)系電話：***/***402/***913/***799/***903/***785/***361/***110/***321 電子郵件：huangqs@zjscs.com 需求聯(lián)系人（僅限于需求內(nèi)容溝通）：梁小姐電話：*** 購買招標文件***：*** 聯(lián)系電話：*** 電子郵箱: zhongjbss2@zjscs.com 開戶銀行：中信銀行廣州花園支行賬號：*** 招標代理機構(gòu)：*** ******月***日 EndFragment
