3.資質(zhì)要求：

①投標(biāo)人應(yīng)提供有效的營業(yè)執(zhí)照或營業(yè)登記證明文件復(fù)印件，加蓋公章；

②非所投設(shè)備制造商的投標(biāo)人須提供制造商的有效授權(quán)書副本(制造商的有效授權(quán)書須為制造商直接開具，正本備查)；

③境內(nèi)投標(biāo)人(除港、澳、***區(qū))須提交有效的銀行基本戶開戶許可證掃描件或基本賬戶證明文件掃描件；

④境內(nèi)投標(biāo)人(除港、澳、***區(qū))應(yīng)當(dāng)提供在開標(biāo)日前 3 個月內(nèi)由其開立基本賬戶的銀行開具的銀行資信證明的原件或復(fù)印件；境外投標(biāo)人應(yīng)當(dāng)提供在開標(biāo)日前 3個月內(nèi)由其開立賬戶的銀行開具的銀行資信證明的原件或復(fù)印件；

⑤投標(biāo)人應(yīng)提供合同簽訂日期為 *** 1 月1 日至今銷售的同類型的設(shè)備＞20臺，要求提供銷售合同復(fù)印件(合同供貨方應(yīng)當(dāng)為投標(biāo)人或其所代理的制造商，合同復(fù)印件能顯示合同簽署日期、銷售貨物名稱***、買賣雙方簽字或蓋章信息)。
